Resident Discharge

- Discharge to Hospital

When we anticipate a resident's discharge, we start making plans soon after admittance. The care team of professionals starts the rehabilitation process with discharge in mind. Anticipating your needs and finding out your plans helps us equip you with the best care possible.

Steps to Discharge

Discharge often depends on the therapist's evaluation of progress and knowledge that the resident has reached his/her maximum potential with skilled care.

We notify the resident and family about discharge.

We contact the resident's physician for the discharge orders, which will include follow-up services at home if needed.

Based on the doctor's order, our rehabilitation coordinator arranges for follow-up home health services and/or special equipment.

Medications. We will ask if you have medications at home and if not, what pharmacy you will use. We will notify your physician of the pharmacy information. Your physician will place the medication order for you.

On the day of discharge, choose a time before 1:00 p.m. that is convenient for all parties. Following Medicare guidelines, we must charge you for another day when check out occurs after 1:00 p.m.
